PRIVACY ACT STATEMENT:
AUTHORITY: 10 U.S.C. 8013, Secretary of the Air Force: powers and duties; delegation by and AFI 38-501, Air Force Survey Program.
PURPOSE: The Air Force Combined Mishap Reduction System (AFCMRS) is a commanders' tool that anonymously collects safety perceptions from organizational members within span of control of the commander to assess the safety culture and improve mission effectiveness.
ROUTINE USES: PII is routinely used to identify the name and contact information of commanders and points of contact to provide the results of the survey.
DISCLOSURE: Disclosure is MANDATORY. Nondisclosure will prevent the commander requesting the data from being provided the results of the data collected.

If you do NOT have a 6-digit access code, follow these steps to obtain one:

- Fill in the fields of the email to indicate your unit, your rank/position, your name, and your phone number.
