Surveys should be administered at the squadron level to ensure greater control and efficiency of the administration process. Squadron-level surveys also help commanders instill a stronger safety climate by encouraging safety reporting and direct feedback.

Step 1. Select which survey (see table below for survey descriptions)

OPS Operations Survey Surveys safety climate perceptions for officer and enlisted rated aircrews performing primary flying duties. MX Maintenance Survey Surveys safety climate perceptions for officer, enlisted and non-contractor civilian members performing primary aircraft and munitions maintenance duties. SUP Support Survey Surveys safety culture for NON-nuclear enterprise support squadrons HHQ Higher Headquarters Survey Surveys officer, enlisted and non-contractor civilian members of Numbered Air Forces and above and Space Force Commands on their perceptions of their support to lower echelon units. Driving Safety Driving Safety Survey Surveys officer, enlisted and non-contractor civilian members on their perceptions of their personal PMV use. MVA Motor Vehicle Awareness Survey Surveys motor vehicle awareness among USAF uniformed members and civilians. NucMsn OPS Nuclear Mission Operations Survey Surveys safety climate perceptions for officer and enlisted rated aircrews on the Personnel Reliability Assurance Program and performing aircraft or missile operations  in support of the Air Force nuclear mission. NucMsn MX Nuclear Mission Maintenance Survey Surveys safety climate perceptions for officer, enlisted and non-contractor civilian members on the Personnel Reliability Assurance Program and performing aircraft, missile, or maintenance operations  in support of the Air Force nuclear mission. NucMsn PRAP SUP Nuclear Mission PRAP Support Survey Surveys safety climate perceptions for officer, enlisted and non-contractor civilian members on the Personnel Reliability Assurance Program and NOT performing aircraft, missile, or maintenance operations in support of the Air Force nuclear mission. NucMsn MED Nuclear Mission Medical Survey Surveys safety climate perceptions for medical personnel with responsibilities for the Personal Reliability Assurance Program in support of the Air Force nuclear mission. MED PT Safety MED PT Safety Survey Surveys safety climate perceptions for medical personnel regarding perceptions of patient safety in the performance of their everyday duties. SW OPS Special Warfare Culture Survey Surveys safety climate perceptions for officer, enlisted and non-contractor civilian members in the Special Warfare Operations career field. ISR OPS ISR Operations Survey Surveys safety climate perceptions for officer, enlisted and non-contractor civilian operators in the ISR career field. ISR SUP ISR Support Survey Surveys safety climate perceptions for officer, enlisted and non-contractor civilian non-operators in the ISR career field. Test Agency Test Agency Survey Surveys safety climate perceptions for officer, enlisted and non-contractor civilian members in the Operational and Developmental Test career fields. MED OPS MED OPS Survey Surveys safety climate perceptions for officer, enlisted and non-contractor civilian members in the medical career field.

Step 2. Enter the maximum number of people who will take the survey.

Step 3. Description of group (for most, this will be "All Aircrew" or "All Maintainers" but could be specific like "Instructors", "Students", "Rotary Wing", etc.)

Step 4. Select "Add to Request List" for each survey needed. When ALL surveys have been added to the list, select the long green button below the list of surveys.
